TERMS & CONDITIONS

Payment Policy

 

  • A credit card is required to secure appointments.  

 

  • 1st Appointment and appointments requiring analysis:  Payment for the first appointment is required prior to treatment due to amount of required quality analysis.  All other payments are required at the time service rendered unless a discounted package is purchased or other arrangements have been made. 

 

  • Monthly Payment Plan:  Payments must be up to date prior to services rendered.  Should an account be delinquent, services will halt and the account is subject to a 10% late fee for amounts overdue.

 

  • Discounts:  A 10% discount is offered for those that purchase a package and payment is in full prior to the first appointment.

 

  • Outcall Appointments:  Health Strong is not responsible for long-distant charges.  It’s the client’s responsibility to ensure the conference company is a viable option prior to 24 hours of the appointment, otherwise the appointment will be charged to the client’s account.

 

  • Supplement Fees:  If supplements are not on hand, most can be ordered and dropped shipped directly to the client.  Shipping, handling, and taxes will be charged to client’s account.   Supplement returns and refunds granted if the product has not been opened and returned within 10 days of purchase. A full refund is granted if the supplement contraindicates health.

 

  • Return Checks:  Accounts are charged a $40 return check fee.  All services will halt until service and return check fees are paid.

 

  • Cash, checks, and Visa, Master Card and Discover credit cards are accepted.                                                                                   

 

Nutrition and Lifestyle Therapy

Cancellation Policy

        

  • Single therapy consultations:   If cancelled less than 24 hours in advanced, the client is charged for the appointment.

   

  • Packaged therapy consultations: The client has one week to make up the consultation without being charged for the appointment in full.

 

  • Intensive 2 week therapy service:  There is no refund for cancellations. Cancellations directly affect the quality of daily intensive service.  It’s recommended that appointments be set for 10 business days without interruption.

 

  • If Health Strong cancels an appointment:  If Health Strong cancels a consultation, it is done 24 hours in advance whenever possible.  Emergency cancellations less than 24 hours in advance promise a 50% discount on a client’s next consultation.

 

Refund Policy      

 

  • All private analysis appointments are non-refundable due to the extensive, quality analysis work required.

 

Other Client Expectations    

 

  • If late for appointments: The consultation time is not flexible and will be shortened unless authorized by the therapist.

 

  • Health history is required and the client agrees to notify Health Strong of any changes in health

 

  • No smoking, or be under the influence of drugs or alcohol during appointments.

 

  • Mutual respect is demanded.  Personal and professional boundaries are honored.  Sexual harassment is not tolerated.

 

Confidentiality

 

All information provided, shared or expressed will be held strictly confidential by Health Strong, LLC.   Under no circumstance will the information you provide be shared with a third party without your written consent.

Health Strong respects all clients regardless of age, gender, race, national origin, sexual orientation, religion, socioeconomic status, body type, and political affiliation, state of health or personal habits.

 

 

 

Community Programs and Educational Seminars

  • 100% of group programs and educational seminars are required prior to services rendered.

  • $25 cancellation fee if cancelled 2 weeks or more before the seminar

 

  • Cancellation within the first 10% of class time refunds 90% of seminar price minus $25 cancellation fee

 

  • Cancellation within the first 25% of class time refunds 75% of seminar price minus $25 cancellation fee

 

  • Cancellation within 50% of class time refunds 50% of seminar price minus $25 cancellation fee

 

  • Cancellation within 75% of class time refunds 25% of seminar price minus $25 cancellation fee

 

  • Cancellation after 75% of class time offers NO refund.

 

  • If Health Strong cancels a meeting/seminar:  Cancellations will be 24 hours in advance.  Emergency cancellations less than 24 hours in advance of a class will promise a 50% discount on the next class.

 

Missed program meetings/seminars 

 

  • Missed program and seminars will be charged in full. A webinar or outline will be provided when available.

 

Other Group member expectations
